Description
Unit: Home Plus RC Hospice
Manager Interviews
Certs Required: BLS, Valid Drivers License, Vehicle insurance.
Experience Required: 1.5 years including Home Health/Hospice. SD Licensure for Social Work required: must be eligible and willing to obtain CSW license; CSW or CSW-PIP required to work. MSW - Masters Degree in Social Work.
Skills Required: Psychosocial support, hospice skills to include: assessment of patient/family needs, resource finding, bereavement support. Ability to collaborate with the IDG (Interdisciplinary Group) through the IDG process. Skills needed to meet both home health and hospice criteria and the regulatory requirements for both.
Patient types:
Hospice and Home Health patients requiring Social Work
This is a FIELD position requiring traveler to commute and provide care within the home setting
Weekly Case Conference Process: coming together and discussing patients that have identified needs – the process/schedule of this is explained during the interview
Fleet Vehicle Assignment: travelers must have their own vehicle/insurance; there are times that they could be assigned a fleet vehicle (for longer distance travel), but this is not always the case
Mileage Reimbursement: will go off current IRS rate - (processed through EPIC system) – mailed directly to traveler – traveler will need to be diligent on what address is given to facility to ensure payment is mailed to the correct address
Monthly Expense Reports: submitted to leadership monthly – discussed during interview.
Visits per Day:
This will vary – typically, up to 5-6 patients seen per 8 hr day – traveler will have autonomy and will manage their own schedules
Average drive time/day: varies
Points system per visit (used more for Nursing) – each visit is weighted in points; each day there is an expectation of 5-6 points/day
Floating:
Between Hospice and Home Health – both outpatient/field positions
Will also provide cross-coverage between Rapid City and Spearfish, approx. 45 min driving time
It is recommended travelers find housing in between (Sturgis, SD/Piedmont, SD)
