Description

We are seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN is responsible for performing comprehensive assessments, developing individualized care plans, coordinating with physicians and the interdisciplinary team, and ensuring high-quality, patient-centered care.

Minimum Requirements

Active RN License (State-specific)
Current BLS/CPR Certification (AHA)
Minimum 1 year of recent Home Health RN experience
OASIS documentation experience required
Acute care or home health experience
Valid Driver's License, reliable transportation, and current auto insurance
Strong assessment, communication, and time management skills
