LPN – Skilled Nursing Care

**Schedule**  
Shift: 8hrs

**Assignment Details**  
Contract Weeks: 29  
Guaranteed Hours: 40

**Requirements**  
- Current Nursing License in the Practicing State  
- Valid Driver’s License  
- Current CPR Certification  
- Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting  
- Proof of current Hepatitis consent/declination and TB vaccination record  
- Knowledge of electronic medical charting and time-keeping software (Homecare Homebase)  

**Roles & Responsibilities**  
- Administer skilled nursing care to patients as prescribed by the physician and in accordance with the established plan of care.  
- Assist in identifying the patient's physical, psycho-social, and environmental needs.  
- Collaborate with the RN to plan, coordinate, and implement patient care.  
- Provide effective communication to patients/families, co-workers, and other healthcare professionals.  
- Assist in educating the patient/family on self-care techniques and assess responses to care.  
- Ensure compliance with infection control measures.  
- Support end-of-life care for Hospice patients and assist with admissions, discharges, and transfers.
