Primary RN - Case Manager

Location - Grand Junction, CO
Term/Duration- 13 weeks

Scheduled hours - 40 hrs

Shift- Mon-Fri 8am-5pm

Responsibilities:

Assess patients and develop individualized plans of care.
Manage symptoms and provide skilled nursing care based on patient needs.
Educate patients, families, and caregivers on disease progression, medications, and caregiving techniques.
Lead and collaborate with the interdisciplinary hospice team.
Coordinate care across home, facility, and community settings.
Maintain accurate and timely clinical documentation.
Participate in occasional weekend and on-call backup coverage.
