Location:Anchorage, AK 99508

Shift: Days (40 hrs/week)

Shift time: Day 5x8-Hour (07:30 - 16:00)

Type: Full-Time Contract / Travel

Job Summary:

The RN Case Manager is responsible for coordinating patient care across the healthcare continuum to ensure optimal clinical outcomes, efficient resource utilization, and a smooth discharge process. This role involves working closely with physicians, nurses, patients, and families to develop and implement individualized care plans.
 

Roles and Responsibilities:

Patient Assessment & Care Planning:

• Evaluate patients’ medical histories, current conditions, and treatment plans.
• Identify patient needs, risks, and barriers to care.
• Develop individualized care plans in collaboration with physicians, nurses, and other healthcare professionals.

Care Coordination:

• Coordinate care among multiple providers, including specialists, therapists, and social workers.
• Ensure smooth transitions between hospital, rehab, home, or other care settings.
• Arrange referrals, follow-ups, and community resources as needed.

Monitoring & Documentation:

• Track patient progress and update care plans accordingly.
• Maintain accurate and timely documentation in electronic health records (EHR).
• Ensure compliance with healthcare regulations and facility policies.

Requirements:

• Must have AK RN license in hand for Auto Offer.
• BLS(AHA)/CPR
• Minimum 2 years of Case Management experience and Case Management/Utilization Review

Note: 2-year acute care hospital inpatient Case Manager experience performing Care Coordination and Admission Reviews, must be current experience within the last 6 months
