Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

• Local Pay: $50–$59/hour (W-2)
• Travel Pay: $2,500–$2,600 gross per week
• Mileage Reimbursement: $0.49/mile (from first patient to last patient)

Weekly Hour Guarantee: 32 Hours (Regular Schedule: 40 Hours per week)
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
On-Call/Weekend Requirement: Every 6th weekend

Position Overview:

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality, patient-centered skilled nursing care to patients in their homes. The RN will assess patient needs, develop and implement individualized care plans, educate patients and caregivers, and collaborate with an interdisciplinary team to promote positive patient outcomes.

Key Responsibilities:

• Perform comprehensive nursing assessments and ongoing patient evaluations in the home setting.
• Develop, implement, and update individualized plans of care based on physician orders and patient needs.
• Administer medications, treatments, wound care, IV therapy, and other skilled nursing interventions as ordered.
• Monitor patient progress, identify changes in condition, and communicate updates to physicians and the care team.
• Complete timely and accurate clinical documentation, including OASIS assessments and visit notes.
• Educate patients and caregivers on disease management, medications, safety, and self-care techniques.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Ensure compliance with agency policies, state and federal regulations, and home health standards.
• Participate in the on-call rotation every sixth weekend as scheduled.

Required Qualifications:

• Active Registered Nurse (RN) license in the State of Ohio.
• Minimum of one year of nursing experience (Home Health experience preferred).
• Experience performing patient assessments and developing individualized care plans.
• Knowledge of OASIS documentation and Medicare home health regulations is preferred.
• Current BLS/CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel throughout the assigned coverage area.

 Base Locations & Coverage Areas (Total 4 openings):

• Danville-24541, VA; Coverage area:  Danville City, Pittsylvania County, very limited coverage in Halifax County (only have two patients in that area currently)
• Virginia Beach, VA, 23464; Coverage Area: Norfolk, Virginia Beach
