Description

Case Manager needed in New York

 

Shifts: Mon-Fri 8-4:30 Days 7.5 hours shifts w/ 1 HR lunch  

No call required

 

Details:

1 year of inpatient Case Management exp needed in SNF/LTC

Trauma Level 2 Facility

Certs Needed: BLS, PRI (Patient Review Instrument) Cert & SCREEN Training Cert

Certifications/Specialty Details: PRI & SCREEN certifications (can be taken online through the School of Albany- typically take a couple hours for each certification; Cost approx. $300/certification) -  

 

Type of Patients:

Inpatient - Common types of patients & job duties: Discharge to Home, Community, or SNF. Obtaining housing, Shelter Packets for Homelessness, Hemodialysis packers, KCI referrals for wounds vacs, and more. 

Experience or 'Must have' skills: Must have 1 year of inpatient case management experience. Prefer prior critical care experience

 

Role: Expected to have knowledge of discharge planning and making referrals. Will typically be working on a MS or Tele unit and will split the floor with a Social Worker. Must be able to function independently on the unit and working collaboratively with an interdisciplinary team and patients for discharge planning. Expected to round on the unit with physicians/teams. Need to have a positive attitude, be able to critically think, and good time management skills. 

Will not be doing Utilization Review (UR) as this is a separate/different dept

 

Case load: Floors typically have 33-40 patients; Most floors are split with 1 CM & 1 SW; Caseload: 17-25 patients. Pilot Floors will have 1 CM & 2 SWs; Caseload: 12-17 patients. 

Need to be able to handle at least 20 patients. Typically placed on Med-Surg or Tele Floors following Internal Med and Family Med. 

 

Department Specific Orientation: 2 days of Hospital Orientation- will include EMR training; 1-2 wks with a preceptor depending on how fast they can be checked off.
