Key Responsibilities

• Provide comprehensive primary care across the lifespan, including wellness visits, preventive care, and management of acute and chronic illnesses.
• Assess, diagnose, and develop individualized treatment plans for patients with common family practice conditions.
• Manage chronic diseases such as hypertension, diabetes, thyroid disorders, hyperlipidemia, and obesity using evidence-based guidelines.
• Educate patients on lifestyle modification, nutrition, preventive health, and treatment plans to promote long-term wellness.
• Order, interpret, and review laboratory and diagnostic tests, adjusting care plans as appropriate.
• Collaborate with physicians and interdisciplinary team members to coordinate patient care and specialty referrals when needed.
• Provide medication management, monitor treatment effectiveness, and ensure safe prescribing practices.
• Maintain accurate, timely, and compliant clinical documentation in the electronic medical record.
• Promote continuity of care through regular follow-up visits and patient engagement.
