THEMA Health Services is seeking a Travel Occupational Therapist for a Home Health assignment in Phoenix, Arizona. Responsibilities

  • Perform comprehensive occupational therapy evaluations and reassessments in patients' homes.
  • Develop individualized treatment plans focused on improving independence with activities of daily living (ADLs) and functional mobility.
  • Treat patients recovering from orthopedic, neurological, cardiac, and medically complex conditions.
  • Provide therapeutic activities, upper extremity rehabilitation, cognitive retraining, adaptive equipment recommendations, and home safety assessments.
  • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
  • Educate patients and caregivers on energy conservation techniques, fall prevention, and home exercise programs.
  • Complete timely documentation while maintaining quality and compliance standards.
