THEMA Health Services is seeking a Travel Occupational Therapist for a Home Health assignment in Phoenix, Arizona. Responsibilities
- Perform comprehensive occupational therapy evaluations and reassessments in patients' homes.
- Develop individualized treatment plans focused on improving independence with activities of daily living (ADLs) and functional mobility.
- Treat patients recovering from orthopedic, neurological, cardiac, and medically complex conditions.
- Provide therapeutic activities, upper extremity rehabilitation, cognitive retraining, adaptive equipment recommendations, and home safety assessments.
- Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
- Educate patients and caregivers on energy conservation techniques, fall prevention, and home exercise programs.
- Complete timely documentation while maintaining quality and compliance standards.
