Travel Speech-Language Pathologist (SLP) – Home Health

THEMA Health Services is seeking a Travel Speech-Language Pathologist for a Home Health assignment in Phoenix, Arizona. Join a compassionate healthcare team dedicated to helping adult and geriatric patients improve communication, cognition, and swallowing function while receiving individualized care in the comfort of their own homes.

Responsibilities

  • Perform comprehensive speech-language pathology evaluations and reassessments in patients' homes.
  • Develop individualized treatment plans to address speech, language, cognitive, voice, and swallowing disorders.
  • Treat patients recovering from stroke, neurological disorders, dementia, traumatic brain injury, and other medically complex conditions.
  • Complete swallowing evaluations and implement dysphagia treatment plans as appropriate.
  • Collaborate with physicians, nurses, case managers, therapists, and caregivers to optimize patient outcomes.
  • Educate patients and families on communication strategies, swallowing precautions, and home exercise programs.
  • Complete timely documentation while maintaining quality and compliance standards.
