Position: Travel Registered Nurse (RN) – Case Manager

Job Summary

We are seeking an experienced Travel RN Case Manager to coordinate and manage patient care throughout the continuum of hospitalization. The ideal candidate will collaborate with physicians, nurses, social workers, payers, and interdisciplinary teams to ensure patients receive appropriate, cost-effective, and high-quality care. The RN Case Manager is responsible for discharge planning, utilization review, care coordination, and facilitating smooth transitions of care.

Key Responsibilities

• Coordinate and oversee patient care plans from admission through discharge.
• Perform comprehensive patient assessments to identify clinical, psychosocial, and discharge planning needs.
• Collaborate with physicians, nurses, social workers, therapists, and other healthcare professionals to develop individualized care plans.
• Facilitate discharge planning, including referrals for home health, rehabilitation, skilled nursing facilities, hospice, durable medical equipment (DME), and community resources.
• Conduct utilization review to ensure appropriate level of care and compliance with payer and regulatory requirements.
• Communicate with insurance providers regarding authorizations, continued stay reviews, and discharge approvals.
• Monitor patient progress and adjust care plans to promote safe, timely, and effective transitions of care.
• Educate patients and families regarding treatment plans, available resources, discharge instructions, and follow-up care.
• Maintain accurate documentation in the electronic medical record (EMR) and ensure compliance with hospital policies and regulatory standards.
• Participate in interdisciplinary rounds and quality improvement initiatives.
• Advocate for patients while promoting efficient use of healthcare resources.
