Position Overview

We are seeking an experienced Inpatient Case Manager RN to coordinate patient care throughout hospitalization, ensuring appropriate utilization of healthcare resources, timely discharge planning, and continuity of care. The RN Case Manager works closely with physicians, nursing staff, social workers, payers, and community providers to achieve optimal patient outcomes.

Key Responsibilities

• Coordinate care for hospitalized (Inpatient) patients from admission through discharge.
• Perform comprehensive patient assessments and develop individualized care plans.
• Conduct Utilization Review (UR) to ensure appropriate level of care and medical necessity.
• Collaborate with physicians, social workers, therapists, and interdisciplinary teams.
• Develop and implement safe and effective discharge plans.
• Coordinate post-acute services, including home health, rehabilitation, skilled nursing, and durable medical equipment (DME).
• Identify and address barriers to discharge.
• Educate patients and families regarding treatment plans and available resources.
• Maintain accurate documentation in the electronic medical record.
• Ensure compliance with CMS, Medicare, Medicaid, and hospital regulatory standards.

Minimum Qualifications

• Active New York Registered Nurse (RN) License
• Bachelor of Science in Nursing (BSN) from an accredited nursing program
• Minimum 5 years of recent clinical RN experience
• Experience in:
  • Inpatient Case Management
  • Care Coordination
  • Utilization Review
  • Discharge Planning
• PRI & Screen Certification (required at submission)
• BLS Certification
• Primary Source Verification of NY RN License through NYS Department of Education (NYSED)
• Strong computer and EMR proficiency
