Required Experience & Responsibilities – Home Health RN (OASIS Required)

Required Experience

• Recent OASIS experience is required.
• Home Health nursing experience required.
• Experience with case management and care coordination.
• Proficiency in wound care, IV therapy, wound VAC management, and other high-tech nursing skills preferred.

Key Responsibilities

• Perform comprehensive patient assessments and develop individualized, evidence-based plans of care.
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and follow-up visits.
• Accurately complete OASIS documentation in compliance with agency policies and State/Federal regulations.
• Serve as the primary Case Manager, managing assigned patient caseloads and visit schedules while meeting productivity standards.
• Implement and evaluate nursing interventions to ensure safe, high-quality patient care.
• Maintain timely, accurate clinical documentation using the agency's electronic medical record system (including McKesson processes, as applicable).
• Provide patient and caregiver education on medications, treatments, disease management, discharge planning, and available services.
• Coordinate care with physicians, therapists, social workers, home health aides, vendors, and other interdisciplinary team members.
• Supervise and delegate appropriate patient care tasks to LPNs and Home Health Aides in accordance with scope of practice.
• Monitor patient progress, modify plans of care as needed, and ensure continuity of care.
• Perform complex clinical procedures including IV therapy, wound care, wound VAC management, infection prevention, and other advanced nursing skills.
• Coordinate patient admissions, transfers, discharges, and required regulatory documentation.
• Participate in interdisciplinary team (IDT) meetings, peer reviews, case conferences, and quality improvement initiatives.
• Complete required Agency, Medicare, State, and Federal documentation accurately and within established timelines.
• Advocate for patients and families to ensure individualized, patient-centered care and optimal clinical outcomes.

Qualifications

• Active Registered Nurse (RN) license.
• Current BLS/CPR certification.
• Strong knowledge of Medicare, State, and Federal Home Health regulations.
• Excellent assessment, communication, documentation, time management, and critical thinking skills.
• Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
