Physical Therapist – Home Health

Position: Physical Therapist
Contract Length: 13 Weeks
Start Date: 08/03/2026
Hours per Week: 32
Shift Length: 8-hour shifts
Schedule: Flexible, Monday–Friday with possible weekends

Assignment Overview

The Physical Therapist will provide home health services across Cambria, Northern Cambria, and Indiana County. This role focuses on delivering high-quality, patient-centered care in a home-based setting.

Key Responsibilities

Perform physical therapy evaluations and treatments in patients’ homes
Develop and implement individualized plans of care
Monitor patient progress and adjust treatment plans as needed
Accurately document visits and clinical findings using HomeCare HomeBase (HCHB)
Collaborate with interdisciplinary care teams
Educate patients and caregivers on exercises, mobility, and safety
Participate in weekend and on-call coverage as required

Travel

Coverage area includes Cambria, Northern Cambria, and Indiana County
Mileage reimbursement provided

Interview Requirement

Client interview required

Minimum Qualifications

Active PT license in state of Pennsylvania
Home health experience preferred
