Job Description
We are seeking an experienced Registered Nurse Case Manager for an 8-week travel assignment at Adventist Health – Lodi Memorial in Lodi, California. The RN Case Manager will coordinate patient care across the continuum, working collaboratively with physicians, nurses, patients, families, and the interdisciplinary team to ensure safe, efficient, and high-quality care from admission through discharge.
Responsibilities
- Coordinate patient care throughout the hospitalization.
- Develop, implement, and evaluate individualized discharge plans.
- Collaborate with physicians, nursing staff, and multidisciplinary teams.
- Facilitate appropriate utilization of healthcare resources.
- Educate and advocate for patients and families.
- Ensure timely discharge planning and continuity of care.
- Maintain accurate clinical documentation.
Requirements
- Active California RN license (Required).
- Recent Case Management experience.
Recent EPIC and Cerner EMR experience within the last 5 years (Required).
