Travel Occupational Therapist (OT) – Skilled Nursing Facility

Morris, Illinois

We are seeking a Travel Occupational Therapist for a 14-week contract in Morris, Illinois. This Skilled Nursing Facility opportunity offers the chance to work with adult and geriatric patients while helping them improve independence, safety, and overall functional ability.

This is a great fit for an OT who enjoys post-acute rehab, building relationships with patients, and working as part of a collaborative interdisciplinary care team.

Responsibilities

Perform comprehensive occupational therapy evaluations and reassessments.
Develop individualized treatment plans focused on improving ADLs and functional independence.
Treat orthopedic, neurological, post-surgical, and medically complex patients.
Provide ADL retraining, upper extremity rehabilitation, transfer training, cognitive interventions, and adaptive equipment education.
Collaborate with nursing staff, physicians, PT, SLP, and case management.
Educate patients and caregivers on safety strategies, home modifications, and discharge planning.
Complete timely documentation and progress reporting.
