Description
8a - 4p Rotating weekends every 4th or 5th 1 summer/1 winter holidy Must have exp. with Sub- acute pts.
2 years of acute care/In Patient experience required
Case Management, Managed Care.
- Coordinates patients through the acute care hospitalization
- Manages all aspects of discharge planning/transition process
- Performs utilization management and quality screenings
- In- Patient Case Management is required
