Job Opportunity: Licensed Clinical Social Worker (LCSW)
Location: Reidsville, North Carolina
Employment Type: Travel/Contract
Contract Length: 13 weeks
Work Schedule
- Monday to Friday, 5 days a week (5x8 hour shifts)
- Shift hours: 8:00 AM – 4:30 PM
- Holidays as needed
- Floating may be required depending on patient population needs
Job Description
- Position in the Transition of Care Department focusing on disposition planning and addressing Social Determinants of Health (SDOH) such as domestic violence, substance abuse, and homelessness.
- Specializes in Skilled Nursing Facility (SNF) placement and home health/Durable Medical Equipment (DME) arrangements for adult Neurology patients, primarily those with Stroke and Seizure diagnoses.
- Opportunity to assist with other patient populations while maintaining core responsibilities.
Required Qualifications
- Valid Licensed Clinical Social Worker (LCSW) license in North Carolina
- Minimum of 1 year of Licensed Clinical Social Worker experience, including experience within the last year
- Basic Life Support (BLS) certification
Compliance and Documentation Requirements
- Government-issued photo ID (Driver’s License or State ID) required.
- Social Security Card required for badge issuance.
- Background check within 60 days of start date covering all locations lived/worked/schooled in the past 7 years, including aliases, social security trace, statewide and national criminal searches, federal court, sex offender registry, and education verification.
- Foreign equivalency evaluation required if education was obtained outside the U.S.
- Employment application/resume with 7 years of work history required.
- Verification of all employment within the last 5 years through a third-party vendor.
- Two supervisory references required from clinical positions, with at least one reference from within 12 months and another within 24 months prior to start.
- Annual skills checklist with a one-week grace period for renewal.
- Facility-specific photo for badge meeting strict requirements (no selfies, solid color background, business casual or uniform attire, forward-facing).
- Completion of a health history form and submission of required immunizations and health documents (see Health Requirements).
- Completion of onboarding tasks including Workday login and multi-factor authentication setup prior to arrival.
Health and Immunization Requirements
- Negative 10-panel drug screen within 40 days of start.
- Fit mask test completed within the current calendar year and updated annually (N95 respirators approved).
- Proof of adult Tdap vaccine.
- Negative TB test (Qgold/TSPOT) within 6 months of start or chest X-ray if history of positive TB.
- TB questionnaire within 30 days of start and updated annually.
- Measles, Mumps, Rubella (MMR) vaccination or positive titer proof.
- Hepatitis B titer or declination with booster series as applicable.
- Varicella vaccination or positive titer proof.
- Seasonal flu vaccine or approved medical/religious exemption required during flu season (Sept 22 – Feb 27).
Testing and Certification Requirements
- Pass/fail EKG (Dysrhythmia) competency exam required for all RNs; not required for Behavioral Health or Case Managers. (Note: LCSW is not an RN, so this does not apply.)
- Acknowledgement of pass/fail testing for EPIC system required.
- Orientation Safety Manual acknowledgement required.
Additional Notes
- Candidate must be flexible with scheduling and aware of zero-tolerance policy for tardiness and unexcused absences.
- Compliance deadline for all documentation is 12 Noon EST the Wednesday prior to the start date.
- Floating policy applies to all agency staff regardless of unit or facility.
- Prior travel experience is not required; first-time travelers are accepted.
This position offers a meaningful opportunity to work with complex adult neurology patients in a supportive acute care environment, focusing on social work interventions that facilitate patient transitions and address critical social needs.
