Description

Provide compassionate, patient-centered hospice nursing care to patients in their homes, assisted living facilities, nursing homes, and group homes.
Perform comprehensive patient assessments and develop individualized plans of care based on patients' goals and clinical needs.
Manage an assigned caseload of hospice patients and coordinate all aspects of their care.
Lead and collaborate with the interdisciplinary care team to ensure high-quality, holistic patient care.
Monitor, assess, and manage pain and other symptoms to maximize patient comfort and quality of life.
Educate patients and family caregivers on disease progression, symptom management, medications, and end-of-life care.
Support patients and families in identifying and achieving their personal goals of care.
Administer medications, treatments, and interventions in accordance with physician orders and hospice protocols.
Accurately document patient assessments, care plans, interventions, and clinical outcomes in the electronic medical record.
Coordinate with physicians, social workers, chaplains, therapists, and other healthcare professionals to ensure continuity of care.
Provide emotional support, guidance, and counseling to patients and their families throughout the hospice journey.
Respond promptly to changes in patient condition and adjust care plans as needed.
Participate in weekday coverage (Monday–Friday, 8:00 AM–5:00 PM) with occasional weekend backup and on-call responsibilities.
Ensure compliance with hospice regulations, infection prevention standards, and organizational policies.
Advocate for patient dignity, comfort, and quality of life while delivering
