Description
Physical Therapist (PT) — Home Health (Contract)
Location: Brunswick, GA (local travel to patient homes)
Overview
Provide in-home physical therapy services to patients across age groups. You’ll evaluate patient needs, develop and update plans of care, deliver skilled treatment, and educate patients/families to improve function and safety.
Key Responsibilities
- Perform start-of-care and follow-up assessments; determine appropriateness for PT services
- Create, document, and revise Plans of Care (POC), progress notes, and related clinical documentation
- Treat neuromuscular, neurological, and orthopedic conditions per referral and evaluation findings
- Educate patients/caregivers on exercises, mobility, safety, and use of assistive devices (walker, cane, wheelchair, braces, etc.)
- Coordinate care with physicians and the interdisciplinary team
- Follow infection control and universal precautions; support quality patient outcomes
Requirements
- Active Physical Therapist license (state license)
- Current CPR/BLS
- Valid driver’s license, reliable transportation, and insurance
- 1–2 years PT experience (acute care or rehab preferred)
- PT degree from an accredited program
- Home Health experience required, including OASIS and Homecare Homebase (HCHB)
Schedule / Work Details
- Monday–Friday (until route/shift is complete)
- 8-hour days
- On-call may be required if requested (based on branch needs)
