Description

Key Responsibilities

Coordinate patient care across the continuum to ensure quality outcomes.
Perform utilization review and support efficient resource management.
Develop and implement safe, effective discharge plans.
Collaborate with interdisciplinary teams to optimize patient care.
Maintain accurate electronic documentation and demonstrate strong computer literacy.

Required Qualifications

Active New York State Registered Nurse (RN) License.
Primary Source Verification of RN License and Registration from the New York State Department of Education (NYSED).
BLS Certification.
PRI and Screen Certification (required at submission).
Minimum 5 years of clinical experience as a Registered Nurse.
Experience in Care Coordination, Utilization Review, and Discharge Planning.
Associate Degree in Nursing (ASN/ADN) from an accredited nursing program.
