Description
Care Manager
Location: Bronx, New York
Employment Type: Acute - Allied
Contract Length: 14 weeks
Job Summary
- Position is for Inpatient (IP) care, not Emergency Department (ED).
- Full-time schedule: 7 hours per day, 35 hours per week.
Required Qualifications
- Valid RN license in New York
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN) from an accredited program
- Minimum 5 years of experience in Acute Care Case Management, including experience within the last year
- Experience as a Care Manager
- Experience with Care Coordination and Utilization Review
- Experience with Discharge Planning
- Basic Life Support (BLS) certification
- PRI and Screen Certification
Preferred Qualifications
- Bachelor’s or Master’s degree in Nursing.
- Leadership experience.
- Case Management Certification.
Additional Information
- Candidate must have PRI and Screen certification at the time of submission.
