Description
Responsibilities
- Coordinate comprehensive discharge planning and care transitions for adult inpatient and emergency department patients.
- Collaborate with physicians, nurses, social workers, therapists, and community resources to ensure appropriate patient care.
- Conduct utilization reviews and assess medical necessity while promoting quality, cost-effective care.
- Develop individualized discharge plans, arrange post-acute services, and facilitate referrals.
- Communicate with patients and families regarding care plans, discharge needs, and available resources.
- Document case management activities accurately in the electronic medical record.
- Float within scope to support Adult Inpatient, Emergency Department, and Pediatric units as needed.
Requirements
- Active Registered Nurse (RN) license (pending license accepted per facility policy)
- Minimum 2 years of recent acute inpatient hospital Case Management experience (within the last 5 years)
- Experience in acute inpatient medical/surgical, emergency department, and/or pediatric settings
- Current BLS certification
- Current ACLS certification
- Epic experience preferred, but not required
- Travel case management experience preferred
