Description

We are seeking an experienced Registered Nurse (RN) Case Manager to coordinate patient care, facilitate safe discharge planning, and ensure appropriate utilization of healthcare resources. The ideal candidate will have acute care case management experience, strong clinical judgment, and proficiency with the Epic EMR system.

Minimum Qualifications:
-Active California Registered Nurse (RN) License
-Minimum of 1+ years of recent Acute Care Hospital Case Management experience
-Recent experience performing:
-Care Coordination
-Discharge Planning
-Utilization of community and post-acute resources
-Acute Care Hospital Case Management experience is required
