Shift: 08:00 AM-04:30 PM
MUST HAVE 1 year Certified Home Care experience.
1 year Case Management Experience.
Please note: Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience.
Weekend rotations are once every 6 weeks.
