Registered Nurse (RN) – Community Living Center

Position Summary

We are seeking compassionate and experienced Registered Nurses to join a highly respected Community Living Center providing exceptional care to adult patients with complex medical needs. This role offers the opportunity to develop lasting relationships with patients while delivering comprehensive nursing care in a collaborative, team-focused environment.

Ideal candidates have experience in long-term care, skilled nursing, rehabilitation, LTAC, Med Surg, or other inpatient settings and are passionate about delivering high-quality, patient-centered care.

Schedule Options

Choose from multiple full-time schedules:

• 5x8 Days: 7:30 AM – 4:00 PM
• 4x10 Days: 7:30 AM – 6:00 PM
• Evenings: 3:30 PM – 12:00 AM
• Nights:
  • 7:30 PM – 8:00 AM
  • 11:30 PM – 8:00 AM

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Administer medications, IV therapies, and physician-ordered treatments.
• Monitor patient conditions and respond appropriately to changes in clinical status.
• Provide wound care, chronic disease management, and patient education.
• Coordinate admissions, transfers, and discharges.
• Collaborate with physicians, therapists, pharmacists, social workers, and interdisciplinary healthcare teams.
• Accurately document patient care in the electronic medical record.
• Delegate responsibilities and supervise LPNs and nursing assistants when appropriate.
• Promote patient safety while maintaining infection prevention and quality standards.
• Deliver compassionate, evidence-based nursing care that supports the highest level of patient outcomes.

Qualifications

Required

• Active Registered Nurse (RN) license.
• Minimum of 2 years of RN experience.
• Graduate of an accredited nursing program.
• Current BLS certification.
• Strong assessment, communication, and critical thinking skills.

Preferred

Experience in one or more of the following:

• Long-Term Care
• Skilled Nursing Facility (SNF)
• Community Living Center
• Rehabilitation
• Long-Term Acute Care (LTAC)
• Medical-Surgical
• Geriatrics
• Post-Acute Care
• Wound Care
• Charge Nurse or leadership experience
