📍 Location: Mount Vernon, IL
📄 Contract Length: 13 weeks
🚀 Start Date: ASAP
⏱️ Hours: 40 hours/week

🕒 Schedule:
• Monday–Friday
• 8:00 AM – 4:30 PM
• Mileage reimbursement provided for all visits

💰 Pay:
• Local Rate: $60/hr
• Travel Package: $2,646/week

Meals: $476/week
Housing: $770/week

📋 Role Overview:
• Provide home health physical therapy evaluations and treatments
• Develop individualized treatment plans for patients in their homes
• Educate patients and caregivers on exercises, mobility, and recovery strategies
• Oversee and coordinate plans of care for PTAs
• Complete documentation and patient assessments
• Collaborate with physicians and interdisciplinary care teams

✅ Requirements:
• Active Illinois Physical Therapist license
• Minimum 2+ years of Physical Therapist experience
• Home Health experience required
• OASIS experience preferred
• HCHB experience preferred
• Strong documentation and patient care skills
