Travel Physical Therapist (PT) – Skilled Nursing Facility

We are seeking a Travel Physical Therapist for a 13-week Skilled Nursing Facility contract in Puyallup, Washington. This post-acute rehab opportunity offers the chance to work with adult and geriatric patients while helping them improve mobility, strength, balance, and overall functional independence.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on patient goals and physician orders.
• Treat orthopedic, neurological, post-surgical, and medically complex patients.
• Provide gait training, balance training, therapeutic exercise, transfer training, and functional mobility interventions.
• Collaborate with nursing staff, physicians, OT, SLP, and case management.
• Educate patients and caregivers on safety strategies and home exercise programs.
• Complete timely documentation and progress reporting.
