• Weekly Hour Guarantee: 40hrs

• Productivity Expectations (points): 30points

• Coverage Areas: Hillsbourough county

• Shift schedule (days, start & end times): M-F 8:00 am to 4:30pm

• On Call/Weekend Requirement: No

• Will Mileage be reimbursed between visits, from first patient to last patient (Y/N)? Yes

• What are the top responsibilities for this position? (Please be as detailed as possible): Admissions, plan of care development, overseeing PTAs, evaluations.

• Must have qualifications/experience: Experience with home health and Homecare homebase

• What skills/attributes are nice to have, and will set a candidate apart? Flexible and teamplayer

• Type of certification required: Masters or Doctorate in Physical Therapy.

• Dress code/scrub color: Any color scrubs
