Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

• Local Pay: $50.00 to $55.00/hour (W-2)
• Travel Pay: $2,400 to $2,500 gross per week
• Mileage Reimbursement: $0.49/mile (from first patient to last patient)

Location: Martinsville, VA, 24112

Schedule details (hours per day/week): 8h/day, 40h/week

Productivity expectations: 30 points

On Call/Weekend Requirement: weeknight/weekend rotating schedule

Coverage Areas: Martinsville, VA; Henry County, VA

Shift schedule (days, start & end times): M-F 0800 - 1630
 

Responsibilities:

• Deliver skilled home health nursing care by conducting comprehensive patient assessments, completing OASIS documentation, managing medications, performing wound care and other skilled procedures, and educating patients to promote safe, effective care and reduce hospital readmissions.
• Coordinate with physicians and the interdisciplinary care team to develop and implement individualized care plans, maintain accurate documentation in Homecare Homebase (HCHB), meet productivity expectations, and participate in the rotating weekday/weekend on-call schedule while providing care throughout the assigned coverage area.

Must have qualifications/experience:

• To work as a Home Health Registered Nurse, you must hold an active RN license, a degree from an accredited nursing program, and clinical experience in an acute care or skilled nursing setting.
• You also need a clean driving record and strong independent assessment skills.

What skills/attributes are nice to have, and will set a candidate apart?

• 1+ years of home health experience, including OASIS documentation and training. Familiarity with EMR and ability to quickly be competent with HCHB documentation system.
• Experience in wound care and IV medication administration and management.
