Schedule: Flexible, field-based schedule
Patient Population: Adult and geriatric homebound patients
Start Date: ASAP
Salary Range: $104,000–$140,300 annually, based on experience
Caseload:
- Approximately 20–21 visits per week when working with a Physical Therapist Assistant
- Approximately 25 visits per week without PTA support
About the Opportunity
We are working with a well-established home health agency serving the Auburn, Kent, and greater King County areas to hire a compassionate and independent Physical Therapist.
This is an excellent opportunity for a PT who values autonomy, flexible scheduling, and meaningful one-on-one patient relationships. You will provide skilled physical therapy in patients’ homes, helping adults and older adults regain mobility, improve independence, and reach their highest possible functional level.
Responsibilities
- Complete initial physical therapy assessments and develop individualized plans of care
- Perform reassessments at least every 30 days or as required
- Provide physician-ordered physical therapy services in patients’ homes
- Perform skilled therapeutic procedures according to the established plan of care
- Evaluate patients’ mobility, strength, balance, safety, and functional limitations
- Assist physicians and the clinical team with evaluating patients’ levels of function
- Recommend and implement revisions to treatment plans as patients’ needs change
- Complete OASIS assessments, reassessments, resumptions of care, transfers, discharges, and change-in-condition documentation
- Select and document appropriate ICD-10 diagnosis codes
- Prepare accurate clinical notes and progress reports within required timeframes
- Communicate with physicians regarding patient progress and recommended treatment changes
- Educate patients, family members, and caregivers on home exercise programs, mobility, safety, and activities of daily living
- Participate in care planning meetings and interdisciplinary care conferences
- Direct and supervise Physical Therapist Assistants and Home Health Aides as assigned
- Participate in staff education, in-service programs, and quality improvement initiatives
Qualifications
- Graduate of a Physical Therapy program approved by CAPTE, APTA, or an equivalent accrediting organization
- Active Washington State Physical Therapist license in good standing
- Valid driver’s license and reliable transportation
- Ability to travel to patient homes throughout Auburn, Kent, and surrounding King County communities
- Strong communication, documentation, organization, and time-management skills
- Ability to work independently and exercise sound clinical judgment
Preferred Experience
- Two or more years of experience as a Physical Therapist
- Previous home health or field-based clinical experience
- OASIS assessment and documentation experience
- Experience supervising Physical Therapist Assistants
