Job Title: Registered Nurse - Case manager
Location: Mount Vernon, NY, 10550
Shift: Days
Shift Length: 5*8 hours
Guaranteed Hours: 37.5 hours per week
Duration: 13 weeks

Position Summary:
We are seeking an experienced RN Case Manager with 2+ years of acute care hospital inpatient case management experience. The ideal candidate will have expertise in care coordination, admission reviews, discharge planning, and utilization management, ensuring patients receive appropriate, timely, and cost-effective care. The role requires close collaboration with physicians, interdisciplinary teams, patients, and families to facilitate safe transitions of care and improve patient outcomes.
Roles & Responsibilities:
- Perform comprehensive inpatient case management, including care coordination, admission reviews, and discharge planning for acute care patients.
- Collaborate with physicians, nurses, social workers, and the interdisciplinary team to develop and implement individualized care plans.
- Conduct utilization reviews to ensure appropriate level of care, medical necessity, and compliance with payer and regulatory guidelines.
- Coordinate post-discharge services, referrals, and community resources to promote safe and timely transitions of care.
- Maintain accurate clinical documentation, communicate effectively with patients and families, and monitor patient progress to optimize quality outcomes.
Skills:
- Strong inpatient case management, care coordination, and discharge planning skills in an acute care hospital setting.
- Experience performing admission reviews, utilization management, and medical necessity assessments.
- Excellent patient assessment, critical thinking, and clinical decision-making abilities.
- Strong communication and collaboration skills with physicians, interdisciplinary teams, patients, families, and payers.
- Proficient in EMR documentation, regulatory compliance, time management, and coordinating post-acute care services.
