Job Title - Medical Social Worker
Duration - 13 Weeks
Shift - Day 5x8-Hour
Location - Mission Viejo, CA 92691
Position Summary:
Medical Social Worker with experience providing comprehensive psychosocial assessments, care coordination, crisis intervention, counseling, and case management services across diverse healthcare settings.
Job Duties:
- Conducted comprehensive psychosocial assessments to evaluate patients' emotional, mental, social, and environmental needs.
- Developed individualized care plans in collaboration with patients, families, physicians, and interdisciplinary healthcare teams.
- Provided crisis intervention, supportive counseling, and emotional support to patients and their families.
Requirements:
- Experience: Must Have 1 year of Experience in Acute care Hospital Setting.
- Certifications: BLS, MSW, CA state license, LCSW or ASW (Pending License Accepted)
