Temp-to-Perm Opportunity | $60/hour

Position Details

• Pay: $60/hour
• Schedule: Monday-Friday, 8:30 AM-5:30 PM (1-hour lunch)
• Assignment: Temp-to-Perm
• Setting: Field-based (conducting in-home participant assessments)

What You'll Do

• Conduct UAS-NY assessments and reassessments
• Complete comprehensive clinical assessments
• Perform medication reviews
• Conduct post-hospital discharge evaluations
• Assist with clinical onboarding of new participants
• Ensure assessments are completed accurately and on time
• Collaborate with the interdisciplinary care team to support quality patient care

Qualifications

• Active New York State Registered Nurse (RN) license
• Previous UAS-NY assessment experience is required
• Strong clinical assessment skills
• Excellent communication and documentation abilities
• Comfortable traveling throughout the community to conduct in-home visits
• Ability to work independently while managing a daily schedule
