Travel Physical Therapist (PT) – Home Health

Silverdale, Washington

We are seeking a Travel Physical Therapist for a 13-week Home Health contract in Silverdale, Washington. This opportunity offers the independence and flexibility of home health while providing meaningful one-on-one care to adult and geriatric patients in their homes.

This is a great fit for a PT who enjoys autonomy, patient-centered care, and helping individuals improve mobility, strength, safety, and overall independence in their home environment.

Responsibilities

Perform comprehensive physical therapy evaluations and reassessments.
Develop individualized treatment plans based on physician orders and patient goals.
Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients.
Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
Complete timely documentation and maintain home health compliance standards.
