Description
Fingerprinting must be submitted and cleared prior to start. ** REQUIRED: Bachelor s degree from the Commission of Accreditation in Physical Therapy Education (CAPTE) accredited school of Physical Therapy. Must possess a valid, current, full and unrestricted license in any state within the United States, District of Columbia, or the Commonwealth of Puerto Rico during tenure of employment. Must have and maintain a current Basic Life Support (BLS) certification by the American Heart Association. Applicant must meet requirements to be credentialed to serve on the Medical Staff. All related information required for credentialing must be provided. Additional Information: ORIENTATION: Every 2 weeks COVID & FLU EXEMPTIONS: May accept after declination form is submitted HOUSING APP: Must be submitted 2 weeks prior to start - form uploaded in Medefis Position Urgency: Quick Start Billing string = Tuba City Regional
