Minimum 2 years ICU experience
Cardiac experience preferred- Post cath intervention, balloon pump, and Impella.
OH or compact license
BLS, ACLS
Must be willing to float to all campuses
Call Required
Travel experience is required

**No holiday RTO will be approved including days during that week. This does not mean your candidate will work every holiday but we cannot guarantee which they will have off.

1. RTO:  Any RTO should be included with submissions. Facility will not accept time off for any major holidays.
2. 2yrs recent/continuous experience
3. Valid state license required. Pending Licenses not accepted
4. Epic Experience preferred
5. Locals accepted at same BR
6. Candidates must be able to float to all 3 campuses (Ontario, Bucyrus, Galion) if there is a need
7. Smoke-Free Facility and Campus
8. On Call: if placed on call, traveler must arrive at the hospital within 1 hour when called in
9. Avita Traveler Expectations Document:
   • Vendors & recruiters must review this document with candidates during the offer stage. It should be noted with submission that the document has been reviewed with the traveler and that the traveler is in agreement. The actual signature can occur during the offer or compliance stage.
10. Work History & References:
    • Please remind vendors & recruiters to place a strong emphasis on thoroughly reviewing each clinicians work history. Any candidates with prior terminations, contract cancellations, or red flags in their employment history (regardless of timeframe) should not be submitted to Avita at this time.
    • Candidate profiles must include two recent supervisory references.
    • Ensure that references are positioned directly after the work history section for review.
11. Modules:
    • Modules are non-billable, up to 8 hours of modules required - factored into NBO
    • Modules are completed pre-start, and vary by profession
    • Modules are recorded through HealthStream
