Responsibilities:

Coordinate patient care with physicians and the multidisciplinary team.
Perform patient assessments, care planning, and ongoing evaluations.
Manage discharge planning and care transitions.
Support utilization management and maintain accurate documentation.

Requirements:

Active California RN License – Required
BLS/CPR Certification – Required
Minimum 1 year of recent RN Case Management/Care Coordination experience – Required
Experience in Acute Care Hospital Case Management – Required
