Description

RN – Case Manager | Roles & Responsibilities

Coordinate care for complex patients across the healthcare system and provide virtual case management services.

Perform comprehensive patient assessments and develop individualized care plans.

Manage care coordination, discharge planning, and transitions of care.

Conduct utilization review, concurrent reviews, continued stay reviews, and medical necessity evaluations using InterQual Criteria.

Coordinate with physicians, social workers, community resources, and interdisciplinary teams to ensure optimal patient outcomes.

Facilitate prior authorizations, DME arrangements, benefits eligibility, and CMS compliance.

Provide telephonic case management, disease management, and patient education.

Maintain accurate documentation in Epic and ensure compliance with HIPAA, Joint Commission, and CMS guidelines.

Float to San Francisco and Emeryville offices as needed while primarily working virtually.

Required Documents / Certifications

  • Active California RN License
  • BLS (AHA or ARC)
  • 2+ years of Inpatient Case Management experience
  • Travel experience required
  • Epic experience
  • Experience with care coordination, discharge planning, utilization review, InterQual Criteria, prior authorizations, concurrent review, and medical necessity
  • Acute care hospital experience (required)
  • Trauma Level I/II experience (preferred)
