Description

Requirements

  • Master's Degree in Social Work (MSW)
  • Minimum 1.5 years of Home Health and/or Hospice experience
  • First-time travelers accepted with required experience
  • South Dakota Social Work License (must be eligible and willing to obtain)
    • CSW or CSW-PIP required
  • Current AHA BLS certification
  • Valid driver's license
  • Personal vehicle and auto insurance required
  • Must live more than 100 miles from the facility
  • Former permanent staff must wait 1 year before returning as a traveler

Job Responsibilities

  • Provide psychosocial assessments and support to home health and hospice patients.
  • Assess patient and family needs.
  • Connect patients with community resources.
  • Provide bereavement counseling and emotional support.
  • Collaborate with the interdisciplinary team (IDG).
  • Meet regulatory requirements for both Home Health and Hospice services.
  • Independently manage daily patient schedule.

Travel & Mileage

  • Field-based position.
  • Use of personal vehicle is expected.
  • Mileage reimbursed at the current IRS rate.
  • Occasional fleet vehicle may be available for longer-distance travel.
  • Coverage may include Rapid City and Spearfish (approximately 45 minutes apart).
  • Recommended housing between the locations (such as Sturgis or Piedmont).

Documentation

  • Epic Remote Client (preferred, not required)
  • Rover mobile app for visit documentation and GPS verification
  • HIPAA-compliant secure messaging

Orientation

  • Approximately 1 week
  • Includes online education and shadowing experienced staff.

Schedule

  • Monday–Friday
  • No routine weekends
  • No scheduled call, although hospice visits may extend if patients experience crises.
  • Time off requires contract approval.
