Job Overview

  • Position: Social Worker – Home Health / Hospice (Field)
  • Shift: 8 Hour Days
  • Schedule: Monday – Friday, 8:00 AM – 4:30 PM
  • Contract Length: (13 Weeks)
  • Guaranteed Hours: 40 hours/week
  • Start Date: ASAP

Facility Details

  • Setting: Home Health & Hospice (Field Position)
  • Location: South Dakota (Rapid City & Spearfish coverage)

Job Responsibilities

  • Provide psychosocial assessments and support for Home Health and Hospice patients.
  • Conduct home visits and manage an independent daily schedule.
  • Assess patient and family needs.
  • Assist with community resource coordination.
  • Provide bereavement counseling and hospice support.
  • Participate in interdisciplinary team (IDG) meetings and weekly case conferences.
  • Meet regulatory requirements for both Hospice and Home Health services.
  • Collaborate with physicians, nurses, case managers, and leadership.
  • Travel to patient homes throughout assigned service areas.

Required Experience

  • Minimum 1.5 years of Social Work experience.
  • Home Health and Hospice experience required.
  • First-time travelers accepted with 1.5+ years of relevant experience.

Required Education

  • Master's Degree in Social Work (MSW)

Required Licensure & Certifications

  • South Dakota Social Work License (CSW or CSW-PIP)
  • Must be eligible and willing to obtain SD licensure.
  • Valid Driver's License
  • Current Vehicle Insurance
  • AHA BLS Certification

Scheduling

  • Monday–Friday

Traveler Requirements

  • Must live more than 100 miles from the facility.
  • Permanent staff must wait 1 full year before returning as a traveler.
