Home Health LMSW – Travel Assignment
📍 Location: Portland, OR
🏥 Facility Type: Home Health
📅 Contract Duration: 13 Weeks
🕒 Shift: Days | 5x8 Hours (8:00 AM – 4:30 PM)
Requirements
- Minimum 1 year of Skilled Home Health experience required
- BLS/BCLS certification required
- OASIS experience required
- Must have reliable transportation
- COVID primary series vaccination one booster required
