Experience Requirements
- Minimum 3 years of Case Management experience
- Recent Case Management experience required
- First-time Travelers: Accepted (if experience requirements are met)
Required Certifications & Licensure
- BSN
- Active California RN License (Temporary License NOT accepted)
- BLS
Job Responsibilities
- Participate in a pilot project supporting liver failure patients
- Manage patients entering through:
- Emergency Department (ED)
- Observation (OBS)
- Inpatient units
- Collaborate closely with a multidisciplinary care team
- Work alongside a Community Health Worker (CHW) to coordinate patient care and discharge planning
