Position: Case Manager
Shift: Days | 5x8 (8:00 AM – 4:30 PM)
Contract Length: 12 Weeks

Requirements

Acute Care Discharge Planning experience required.
Utilization Review experience not required for this role.
Experience with Cerner EMR preferred.
Experience with Allscripts, WellSky, or CarePort preferred.
Ability to assess patients for post-acute care needs.
Experience coordinating insurance-related discharge planning.
Ability to deliver IMMs (Important Message from Medicare) and MOON (Medicare Outpatient Observation Notice).
Strong communication, care coordination, and documentation skills.
