Job Title: Travel RN - Home Health

Coverage Areas: Newburgh, NY Territory

Schedule: 40 hours/week

Productivity Expectations: 30

On-Call/Weekend Requirement: Yes

Mileage Reimbursement: Mileage is paid from the first patient to the last patient.

Key Responsibilities:

• Perform 30 productivity points per week by completing Start of Care (SOC), admissions, routine skilled nursing visits, recertifications, resumptions of care, and discharge assessments.
• Provide comprehensive home health care, accurate OASIS documentation, and timely charting in compliance with Medicare and agency standards across the Newburgh, NY territory.
• Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary team.
• Provide skilled nursing interventions, medication and disease management, wound care, and patient/caregiver education.
• Participate in the on-call rotation and ensure high-quality, patient-centered care.

Must Have Qualifications/Experience:

• HCHB experience is a must.
• 1-2 years of home health experience.

Type of Certification Required:

• Active & unrestricted RN license.
• Valid driver's license & reliable transportation.
• Current BLS certification.
