Location: Bronx, NY - 10457
Schedule: Day Shift | Monday–Friday | 8-hour shifts (37.5 hours/week)
Contract Length: 13 Weeks with possible extension

Job Overview

We are seeking an experienced Registered Nurse (RN) Case Manager for a 13-week travel assignment at a leading acute care hospital in Bronx, NY. This role focuses on inpatient discharge planning and care coordination, working closely with physicians, social workers, and the interdisciplinary care team to ensure safe and timely patient transitions.

Responsibilities

• Coordinate inpatient discharge planning and patient care transitions.
• Collaborate with physicians, social workers, and other healthcare professionals.
• Assess patient needs and arrange appropriate post-discharge services.
• Make referrals to home care, skilled nursing facilities, dialysis, wound care, and community resources.
• Participate in interdisciplinary rounds and develop discharge plans.
• Maintain accurate documentation in the electronic medical record.
• Manage a caseload of approximately 17–25 patients while ensuring quality patient outcomes.

Requirements

• Active RN license.
• BSN required.
• Minimum 1 year of inpatient Case Management experience.
• Current BLS Certification.
• PRI (Patient Review Instrument) Certification.
• SCREEN Training Certification.
• Strong knowledge of discharge planning and care coordination.
• Experience in an acute care setting preferred.
• Familiarity with CarePort, MCG, or InterQual is preferred.
• Critical care experience is a plus.
