• Position: RN Case Manager – SNF/LTC

• Weekly Pay: $2,724/week
• Local Pay: $58/hr Regular | $86/hr OT

• Shift: Monday–Friday, Days | 8:00 AM–4:30 PM

Requirements:

• Certifications: BLS, PRI (Patient Review Instrument) Certification, SCREEN Training Certification
• PRI certification required at submission
• SCREEN certification can be completed after offer (online through School of Albany; approx. $300/certification)
• Minimum Experience:
  • 1 year inpatient case management experience required
  • Preferred: Critical care experience
  • 1 year complex patient experience managing multiple chronic illnesses, behavioral health needs, or social challenges

Role Details:

• Responsible for discharge planning and referrals to home, community, or SNF settings
• Works primarily on Med-Surg or Tele units alongside Social Workers
• Collaborates with physicians, interdisciplinary teams, and patients
• Participates in daily rounds and discharge planning
• Must be able to independently manage caseloads and prioritize patient needs
• Will not perform Utilization Review (handled by a separate department)

Patient Assignment:

• Typical floor census: 33–40 patients
• Standard assignment: 17–25 patients per Case Manager
• Pilot floors: 12–17 patients per Case Manager (with additional Social Worker support)
• Must be able to manage approximately 20 patients

Orientation:

• 2 days hospital orientation (includes EMR training)
• 1–2 weeks with preceptor based on competency progression
