Job Summary:
We are seeking an experienced Registered Nurse (RN) to provide direct patient care in a Home Health and/or Hospice setting. The RN will perform assessments, administer medications and IV therapies, manage pain, provide patient and family education, and coordinate care with the multidisciplinary healthcare team.

Responsibilities:

• Provide direct nursing care to assigned Home Health/Hospice patients.
• Perform comprehensive physical assessments and develop individualized care plans.
• Administer medications, IV/infusion therapies, and other nursing interventions.
• Provide pain management and address patients’ nutritional and other care needs.
• Collect specimens and perform necessary clinical procedures.
• Educate patients, families, and caregivers regarding treatment, medications, and home care.
• Monitor patient progress and adjust the nursing plan of care as needed.
• Assist with discharge planning and coordinate care with other healthcare disciplines.
• Apply evidence-based nursing practices and follow applicable clinical, ethical, and legal standards.
• Maintain accurate and timely patient documentation.

Requirements:

• Home Health: Minimum 2 years of acute clinical nursing experience + clinical Home Health experience required.
• Hospice: Minimum 2 years of acute clinical nursing or Home Health experience required.
• Hospice nursing experience preferred.
• Associate Degree in Nursing (ADN) or higher required.
• Current California RN License required.
• Current BLS certification required.
• Valid driver’s license and reliable, insured vehicle in good working condition required.
• Strong patient-care, assessment, communication, IV therapy, medication administration, and documentation skills.

Important: Candidates must have the required Home Health/acute nursing experience, current California RN license and BLS, and a reliable vehicle for home visits.
