Description

Location: Mendota, IL

Contract Length: 13 Weeks

Schedule: Monday–Friday, 40 hours per week

Shift: Day Shift with Weekend & Holiday Rotation

We are seeking a dedicated and compassionate Physical Therapist (PT) for a 13-week contract assignment in Mendota, Illinois. This is an excellent opportunity to join a collaborative healthcare team while providing high-quality patient care in a rewarding clinical setting.

Responsibilities

Evaluate, develop, and implement individualized physical therapy treatment plans.
Assess patient progress and modify treatment plans as needed.
Collaborate with physicians, nurses, and interdisciplinary team members to optimize patient outcomes.
Educate patients and caregivers on treatment plans, exercises, and injury prevention.
Maintain accurate and timely clinical documentation.
Participate in weekend and holiday rotation as scheduled.

Requirements

Minimum 1 year of Physical Therapist experience required.
Current Physical Therapist (PT) License.
Current BLS (Basic Life Support) certification.
Strong communication, organizational, and patient care skills.
